Arrive Ahead. Please plan to be 15 minutes early to your appointment to sign in and update your health information on file.
Please let us know ahead of time if you have high blood pressure, diabetes or are pregnant.
Cancel Appointments 24 hrs in Advance. Please give us 24 hours notice if you need to cancel or reschedule your appointment. Appointments not canceled 24 hours in advance may be suject to a charge of 50% of the appointment fee and may be billed automatically to your account.
